Notice of Informed Consent

I understand that BaySport staff will have access to my test results for ascertaining if the results are within normal ranges. Group statistics may be used by BaySport and shared with IBEW 595 representatives for summary report purposes. Personal health information and test results may be shared between BaySport staff solely for the purpose of additional and specific follow-up, results counseling services and targeted educational outreach. No other individual or entity will have access to your individual test results without express written permission from you or via court order. The responsibility for initiating any medical examination for possible abnormalities identified by this Wellness Screening program is yours. Neither BaySport, IBEW 595 nor its contractors has any such responsibility.
