Welcome to the registration for the health screening event at Eskaton - Care Center Greenhaven! Please log in if you have already created an account or create a new user profile by selecting the “I want to register” button below. Once you have an account, please answer some basic health questions (3-5 minutes) and you will be ready for your health screening.

Health screenings will include:

· Total Cholesterol, HDL Cholesterol, LDL Cholesterol, Triglycerides, TC/HDL Ratio, and Non-Fasting OR Fasting Glucose

· Blood Pressure

· Body Composition

· Body Mass Index

· Results review – Brief consulting with Clinical Health Appraisals staff. Each participant will be emailed their individual results.

Your individual health responses or Biometric Testing data will NOT be shared with your employer.

Please arrive on time for your health screening appointment. This is preferably a fasting test, so please try and consume 0 calories (water, black coffee and unsweetened tea are okay) for at least eight hours before your appointment.
