Welcome to the application page for Fresh & Fit, presented by Indiana University Health! We're glad you're interested in taking the next steps toward a healthy lifestyle by applying. The 10-week program will begin the week of August 28 and will last until November 10.

Applications must be submitted by August 18 in order to be considered. Participants will be selected based off of their application responses. Please answer all questions accurately and to the best of your ability.

If you are selected for this program, it is a benefit provided to you, free of cost, with NO requirement for you to receive future services at IU Health. You are NOT required to have any connection to IU Health or an IU Health physician.

For those who apply for Fresh & Fit and are not selected, the information you provided in your application will not be shared with any other parties, neither within IU Health nor externally.

*Please note that the program is not open to former Fresh & Fit participants, IU Health team members or their immediate family members. Applicants must be 18+ years of age.
