Welcome to the application page for Fresh & Fit, presented by Indiana University Health! We're glad you're interested in taking the next steps toward a healthy lifestyle by applying. The FREE 10-week program will begin the week of August 25 and will last until November 7.

Applications MUST be submitted by Monday, August 11 in order to be considered. Participants will be selected based off of their application responses. Please answer all questions accurately and to the best of your ability.

After applying, please periodically check your email for notifications about your application status. You will be notified via email if you have been chosen to move on to the next round of the selection process. Selected participants will then receive a link to schedule their preliminary health screening, which will occur the week of August 25.

If you are selected for this program, it is a benefit provided to you, free of cost, with NO requirement for you to receive future services at IU Health. You are NOT required to have any connection to IU Health or an IU Health physician. For those who apply for Fresh & Fit and are not selected, the information you provided in your application will not be shared with any other parties, neither within IU Health nor externally.

***Please note that the program is not open to former Fresh & Fit participants or current IU Health team members. Applicants must be 18+ years of age.
